New Students

This hospital clinical day will be at least 1 of the 4 clinical days and is provided at no additional charge, we are trying to provide additional experiences outside the typical long tern nursing facilities. The needed documents will need to be completed and submitted directly to me for review and approval. 

1. The hospital quiz/signature pages and the OSHA paperwork for the clinical portion of the program which was discussed in the online orientation. For the hospital quiz/signature pages, you will need to answer the questionnaire with 100% accuracy. Once you have completed the quizzes and paperwork, email them back to me at [email protected] and I will review them for any corrections. Please do not print out the entire booklets, there will be 10 pages that you will need to submit back to me, the last 5 pages from each hospital orientation booklet. Any pages that require your signature, will need printed off for your actual signature to be put on them, if you are under the age of 18, your parent/guardian will also need to co-sign the signature pages. All 3 of these items can be found within the online orientation you took, if you encounter any problems, please let me know
   
   
2. You will also need to complete a urine drug test and N-95 mask fitting, both at no cost to you. This can be scheduled here. Please make sure to fill out and bring the OSHA N95 mask fit health questionnaire with you to your appointment. If you choose to have the urine drug test at an outside testing facility, it will need to be a 10 panel or higher and will be at your cost. If you are under the age of 18, your parent/guardian will need to be with you at this appointment.
